Quality of Life Group Case Management Internship Program Description

The Organization

Founded in 2009 by Dr. Min Cole, Ph.D. in Applied Gerontology, the Quality of Life Group (QOLG) is a
nonprofit and nonpartisan organization, with the mission of working towards the best quality of life for
older adults and their families by providing innovative person-centered programs, services, research, and
education in Long-Term Support and Services (LTSS) in the communities we serve. Please go to

Essential Duties and Responsibilities:

QOLG Case Management program aims to provide locally or remotely help for family caregivers and
people they care for with complex issues and decision-making connected to aging and disabilities.

• Assessing the client’s current medical, psychological, emotional, functional, cognitive, and social
needs and conditions and collecting information by interviewing and/or making on-site visits.
• Assess the client’s caregiving condition by interviewing families, caregivers, neighbors, and other
related resources.
• Developing long-term care written strategic planning and setting reasonable and sustainable goals.
• Helping the clients by referring him/her to community resources; arranging for appointments;
establishing rapport with other agencies as needed.
• keeping records of each case by documenting the client’s situation and the client’s actions in the
progress notes.
• Monitoring the implementation of the plans by periodic follow-up and telephone reassurance.
• Updating the federal, state, and local legal requirements by studying existing and new legislation;
enforcing adherence to requirements; and reporting to the organization management team on
planning and actions.
• Maintains client confidence and protects operations by keeping information confidential under
HIPAA.
• Attends, participates, leads, and/or facilitates routinely scheduled meetings and education sessions as
required for the program or agency.
• Maintains a strong working relationship with all other programs to keep lines of communication
open regarding clients and referrals.

• Acts as client advocate interfacing with Social Security, Medicare/Medi-Cal, and other agencies.
Ensures clients receive all benefits to which they are entitled.
• Performs additional duties as assigned.

Job Qualifications:

• Must be accountable, reliable, and able to relate well to the older adults and work closely with the
family members and caregivers.
• Possess knowledge of social, economic, and psychological needs of homebound elderly and the
needs of their family caregivers.
• Exceptional listening skills, a collaborative style coupled with adaptability and flexibility, high
energy, and the ability to prioritize.
• Organized, with an ability to prioritize time-sensitive assignments.
• Must be a team player and able to work closely with other case managers, and supervisors,
participating in team meetings and required training.
• Must be able to communicate in English and preferred in other languages as well.
